Admission

When You Arrive at the Wellington Street Campus
Please report to the Admission Centre on Level 2, North Block at the scheduled time.  You will need to advise the Admission Centre staff if you have been hospitalised in the last seven days or have been admitted to a hospital outside W.A. within the past 12 months.

When you arrive at the Shenton Park Campus
Direct admissions and patients for elective surgery are admitted by the Admission Clerk, who is located at the main entrance to the Outpatients building.  After hours, admissions are taken at the main entrance reception desk.

Please be sure to advise medical and nursing staff of any allergies you have to antibiotics and other medications, food or any other known allergies, e.g. adhesive tapes.

What to bring:
Please be sure to bring with you:

  • Your admission form.
  • Your Medicare card and your private hospital insurance card, if you have one.
  • If you are covered by Workers Compensation or the Motor Vehicle Insurance Trust, you should bring details of your employer and the date, time and place of your accident.
  • The name and address of your general practitioner.
  • Emergency contact details of your next of kin.
  • Any medications you may be taking and x-rays.
  • Pyjamas or nightgowns, dressing gown and slippers (non-slip sole).  It is important that footwear is worn at all times for safety and hygiene purposes.
  • Personal toiletries such as soap, toothbrush, toothpaste, comb, shaving kit, tissues, etc.
  • A small amount of money for items such as newspapers, magazines, telephone calls, hire of television, etc.

We request that you do not bring jewellery or other valuables or excess cash when you are admitted to hospital.

We will not be responsible for any loss of valuables or cash kept in the ward.


Discharge

During your stay, the doctor will advise when you can expect to be discharged home.

Even before this happens, we believe it is in your best interest to start planning for your discharge and after-care as soon as possible.

If you think you will need support at home, please talk to the staff caring for you so that the required support can be arranged ahead of time.

Discharge from the hospital is before 11:00 am.  Patients are responsible for making their own transport arrangements to and from the hospital.

Please ensure you have collected all your personal items.  Before leaving the ward, please make sure you collect any private x-rays or medications and confirm if you have any follow-up appointments.

At the Wellington Street Campus, once you are ready for discharge, you may be transferred to a comfortable Transit Lounge on Level 3, North Block, to wait for your relative/carer or friend to collect you.
